Northland Cares (formerly known as the Sliding Fee Scale) offers a simplified and affordable approach to healthcare costs, making medical, behavioral, and dental services more obtainable to individuals and families.

Eligibility

Northland Cares eligibility is based on individuals and families who meet specific income and family size criteria. You may qualify with or without insurance, and you may qualify even though you have Medicare or Medicaid.

To see if you are eligible, find your family size in the information below. If your family’s annual income is less than the amount listed, you will most likely qualify for Northland Cares.

Apply

To apply for Northland Cares, please submit one acceptable proof of income for all members of your household
(related and unrelated) listed below.

  • Two most recent pay stubs
  • Current Tax Return
  • Current W-2s from all employment
  • Social Security or Unemployment Award Letter
  • Department of Social Services Benefit Statements
  • Letter from a government office explaining the benefits you do or do not qualify for (unemployment office or social security office, etc.)

Northland Cares Application

Northland Cares applications are available at any of our locations or available to apply online. All information will remain confidential and will only be viewed by designated Northland Health Centers staff.

Along with proof of income, please also provide a completed Northland Cares application. 

Q&A

If you or anyone in your household does not have a source of income, you will need to get ahold of the Northland Cares administrator at 701-448-2054 or email cares@northlandchc.org and attest to not having an income source.

Household size is defined as anyone who is financially responsible for one another. This may include immediate or non-immediate family members depending on your own personal situation. 

Adult roommates who have separate finances are not to be considered in the same household, even if they are living together.

Northland Cares can be used in conjunction with ANY insurance. As it is solely based on your household size and income.

The Northland Cares Program can be active the same day it is submitted, as long as all required documentation is sent in. Please allow up to 30 days from the time of submitting your application for processing and official approval. 

All visits provided at a Northland facility are eligible to be covered. This includes Medical services, Dental services, Behavioral Health Services, as well as all Northland telehealth services.

All patients who do not pay their share of the bill will receive a statement. We do accept payment plans. Amounts outstanding on your account may be referred to a collection agency for further action, so it is important to keep your account up to date. If you need to set up a payment plan, please contact the Billing Department at 877-609-3577 Option 2 or email billing@northlandchc.org.
